EDA participates in care
insurance plans such as preferred provider organizations (PPO), health
maintenance organizations (HMO), point of service products, independent
practice associations (IPA) and Medicare. If you are in doubt as to
whether we participate with your plan, please contact our office to
confirm. Many of these managed care plans have referral guidelines.
It is the responsibility of the patient to follow those guidelines before
seeking specialty care.
If appropriate referrals
are not in place at the time of the appointment, you will be expected
to pay for your visit at the time of service. EDA will also expect you
to pay your co-payment at the time of service. If you do not have a
managed care plan, EDA will file your commercial insurance for you.
We will file your claim as a courtesy; however, if our clinic files
a claim for you, benefits will be assigned to the provider. We will
expect the patient to pay for any unmet deductible and coinsurance at
the time of service. If you have paid for your deductible at the time
of service, we will file a claim to your insurance carrier so that you
may receive deductible credit.
Once EDA has filed your
insurance for you, you will not receive a statement from our clinic
until all of your insurance for that claim has been exhausted. When
we receive notification from your insurance company that your claim
has been processed ( paid or denied), you will receive a statement.
In the event that your insurance requests additional information from
you, please respond promptly. If you do not comply with your insurance,
the bill will be your responsibility.
It is very important that
you bring your insurance card to every appointment. If you do
not have insurance, we will be happy to make financial arrangements
PRIOR to the visit. We accept cash, Visa, Mastercard and American Express.
We frequently provide services that are not covered by your insurance.
If we provide non-covered services to you, we will notify you in advance
and have you sign a statement stating that you are aware such services
are not covered by you insurance. We will collect for these non-covered
services at the time they are provided to you. Although such services
are not covered by your insurance, in our opinion, they are medically
necessary and should be a part of your care plan.
