Lab Results, and Prescriptions
Medical Records
In order to have your medical records released, a written request signed
by either you or your legal guardian is required. Please ask for a standard
release form from out office and allow 7 - 10 business days for processing.
Lab Results
Lab results are typically available within 7 - 10 business days and will
be promptly mailed to the address we have on file. If your doctor sees
the medical necessity, he will call you in advance of mailing the results
to discuss the findings or change in treatment. Please do not call the
office unless instructed by your physician.
Prescription Refills
In order to receive timely prescription refills, please have your PHARMACY
call us with the request two or three days in advance of running out of
your medication. We do not accept patient called requests for prescriptions,
unless the situation is an emergency.
